=== Microstructure and crack propagation === During the dentinogenesis process, the odontoblast cells retreat from the DEJ to the outer lining of the pulp, leaving behind microtubules filled with cytoplasmic extensions and depositing intertubular dentin (ITD) in its place. ITD comprises the bulk of the dentin and, similarly to bone, is a matrix composite of tablet-shaped hydroxyapatite nanoparticles wrapped around collagen fibers. The mineralized collagen fibers are arranged in layers oriented perpendicular to the direction of the dentin microtubules which are lined with peritubular dentin (PTD), a 1-2 μm thick layer of hydroxyapatite tablets with no preferred orientation and lacks any supporting collagen fibers. The hydroxyapatite tablets within the ITD were found to be compressed along the crystallographic c-axis due to tight interaction between the tablets and the collagen fiber. Tablets aligned parallel with the collagen fibers experience a significant increase in compressive stress of around 90 MPa and, for crack formation to occur, tensile stresses must first overcome this residual compressive stress. Since typical mastication stresses do not exceed 40 MPa, the ITD prevents cracks from forming during normal daily use and helps deflect cracks perpendicular to the dentin tubule and away from the pulp. Inelastic deformation of dentin primarily happens through microcracking. Crack propagation within dentin travels preferentially along the interfaces of the ITD layers.

== Predicting response == Trastuzumab inhibits the effects of overexpression of HER2. If the breast cancer does not overexpress HER2, trastuzumab will have no beneficial effect (and may cause harm). Doctors use laboratory tests to discover whether HER2 is overexpressed. In the routine clinical laboratory, the most commonly employed methods for this are immunohistochemistry (IHC) and either silver, chromogenic or fluorescent in situ hybridisation (SISH/CISH/FISH). HER2 amplification can be detected by virtual karyotyping of formalin-fixed paraffin embedded tumor. Virtual karyotyping has the added advantage of assessing copy number changes throughout the genome, in addition to detecting HER-2 amplification (but not overexpression). Numerous PCR-based methodologies have also been described in the literature. It is also possible to estimate HER2 copy number from microarray data. There are two FDA-approved commercial kits available for HER2 IHC; Dako HercepTest and Ventana Pathway. Fluorescent in situ hybridization (FISH) is viewed as being the "gold standard" technique in identifying patients who would benefit from trastuzumab, but it is expensive and requires fluorescence microscopy and an image capture system. The main expense involved with CISH is in the purchase of FDA-approved kits, and as it is not a fluorescent technique it does not require specialist microscopy and slides may be kept permanently. === Continued unrest === Over Christmas, the UDF called for supporters to observe a "Black Christmas" – a form of consumer boycott, among other things – to mourn the casualties. === Cognitive impairment and behavioral disturbances === Persons affected are cognitively impaired and have behavioral disturbances that emerge between two and three years of age. The uncontrollable self-injury associated with LNS also usually begins at three years of age. The self-injury begins with biting of the lips and tongue; as the disease progresses, affected individuals frequently develop finger biting and headbanging. The self-injury can increase during times of stress. Self-harm is a distinguishing characteristic of the disease and is apparent in 85% of affected males. The majority of individuals are cognitively impaired, which is sometimes difficult to distinguish from other symptoms because of the behavioral disturbances and motor deficits associated with the syndrome. In many ways, the behaviors may be seen as a psychological extension of the compulsion to cause self-injury, and include rejecting desired treats or travel, repaying kindness with coldness or rage, failing to answer test questions correctly despite study and a desire to succeed, and provoking anger from caregivers when affection is desired. Compulsive behaviors also occur, including aggressiveness, vomiting, spitting, and coprolalia (involuntary swearing). == Dosage and administration == Each benzhydrocodone/APAP immediate release tablet contains 6.12 mg of benzhydrocodone (equivalent to 6.67 mg of benzhydrocodone hydrochloride) and 325 mg of acetaminophen. The tablets are white, capsule-shaped and are debossed with "KP201" on one side. Dosing should start at one to two tablets every four to six hours, as needed for pain. The dosage should not exceed 12 tablets in a 24-hour period.
